By clicking submit I hereby authorize a deduction in the amount of $5.00 monthly from my pension check to cover my membership dues for Los Angeles Retired Fire & Police Association®. If at any time the amount of said dues should be changed by the Board of Directors of the Los Angeles Retired Fire & Police Association®. I hereby authorize the deduction from my pension and the payment to the Los Angeles Retired Fire & Police Association®. for this purpose, such sum as may be specified by the Board of Directors of Los Angeles Retired Fire & Police Association®. This Authorization shall be effective until cancelled by me.
